Accident report

After an accident: take photos, say what happened.

Photograph the cars, the plates and the damage, and say what happened. AI writes the accident report, following the European Accident Statement, and puts the other driver's name, plate, insurer and policy number in their fields. Before you finish, it lists what's missing. Like the European Accident Statement, it says it's a record of facts, not an admission of liability. In Switzerland, that form is called the Europäisches Unfallprotokoll.

    The PDF is sealed with a trusted timestamp and a code anyone can check. If you want, the other side signs on your phone, on their own phone from a QR code, or later by email.

Accident report: common questions

Do I have to call the police in Switzerland?

If anyone is hurt, yes, at once (117). For damage to property only, tell the person whose property was damaged right away and give your name and address; if that isn't possible, inform the police (Art. 51 SVG).

Is this the European Accident Statement?

No. It follows the same structure (what happened, the other party, damage, injuries, witnesses), but it's Fiveover's report, not that form. If the other driver hands you a European Accident Statement, you can fill it in as well.

Does the other driver have to sign?

No. Their signature is offered, never required: they sign on your phone with their name and finger, on their own phone from a QR code, or later from an email link. Without it, the PDF carries only your signature.
